Liability Waiver:

I hereby consent that the participant may participate in a program  provided by Lifestyle Aquatics, and I hereby execute the agreement, waiver, and release on my/his/her behalf. I hereby assert that the participant is physically able to participate in the selected program.  I hereby agree to indemnify and hold harmless any instructors and Lifestyle Aquatics from any loss, liability, damage, cost, or expense, which may occur as a result of death or injury, or property damage that the participant may sustain while participating in the selected program provided by Lifestyle Aquatics. It is understood that participation in the selected program involves an element of risk and danger of accidents, and knowing those risks, I hereby assume those risks. If any person accompanying the registered participant, including but not limited to, family members, friends, and acquaintances experiences any personal injury while on the premises at any time including during the time of the selected program of the registered participant, neither I nor that person shall be able to pursue any legal action against our instructors, volunteers, admin team or any persons associated with Lifestyle Aquatics.

 

 

Photo Realease:

I hereby grant Lifestyle Aquatics permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration.  In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

Attendance:

In our efforts to minimize distraction during our swim lessons, if the participant is not present 20 minutes after the scheduled start time, the lesson will be lost and the student will not be allowed to join the class. Swim lessons will not be extended if the participant is tardy. If the instructor is tardy, arrangements will be made to make up missed time.

 

Pool Closings/Lesson Cancellations:

In the event that Lifestyle Aquatics has to cancel swim lessons due to pool maintenance or inclement weather, which includes thunderstorms or other dangerous conditions, participants will have the option to come to an alternate class time to make up the canceled lesson. Make up swim lessons will also be scheduled on Fridays. All make up classes need to be scheduled and approved before arrival to ensure we have the proper amount of instructors available.  All notifications concerning lesson cancellations will be communicated via email up to an hour before class time.
